Regulations and Standards
A clear knowledge of regulations and standards regarding medical technology is vital to be able to provide full support to the equipment. Most regulations and standards pertain to patient and staff safety or to performance or manufacturing requirements.
These guidelines may be local (municipal, State or Provincial), or they may be national or international.
Local standards are too variable to cover here, but generally, local governments look to national and international bodies for guidance concerning such topics as medical technology.
National regulatory bodies include the Food and Drug Administration in the United States and Health Canada in Canada. Testing and standards agencies such as Underwriters Laboratories or the Canadian Standards Association carry no legal weight themselves, though their guidelines often form the basis of legal requirements.
Internationally, the Association for the Advancement of Medical Instrumentation (AAMI), the American National Standards Institute (ANSI), the Institute of Electrical and Electronics Engineers (IEEE) and the International Organization for Standardization (ISO) provide information and guidance on many technical matters.

In-vitro tests
In vitro diagnostics (IVDs) are tests that can detect disease, conditions and infections done on samples such as blood or tissue that have been taken from the human body. They allow doctors to diagnose patients effectively and work to provide appropriate treatments.
The samples are analysed directly at physicians' offices or in laboratories, which we supply with our tests and instruments.
Instruments
Most of our in vitro tests run on diagnostic instruments that we develop and manufacture ourselves or with external partners. Some of these devices are small, and are used to analyse a patient's blood or urine. You can find these in doctors offices and clinics. Other instruments are large analysis modules, some the size of small cars, which can be integrated to provide a seamless molecular diagnostics solution for central laboratories. These systems allow hundreds of samples to be analysed in parallel, testing for many different diseases.
The laboratory-based diagnostic systems work with advanced technologies equally as sophisticated as those you might find in modern aircraft. These analytical laboratory instruments are fully automatic, enabling the analysis of a large number of samples in a short time. In addition, advanced automation reduces the opportunity for error, which benefits patients. Thus, laboratories are able to deliver high-quality patient results in a highly efficient manner.

Gallery of real life problems
The following are some examples of equipment problems encountered during the course of my work as a hospital BMET. Many issues are not amenable to photographs, but these images might give the reader a feel for some of the things they could see in the workplace.

This is the AC power cord of an infusion pump. Continual flexing of the cord eventually caused the insulation to split right at the end of the strain relief. While the insulation on the individual wires inside is still intact, this presents a potential shock hazard should further damage occur. The cord was replaced… a simple matter of undoing two restraining screws, removing the cord restraint, replacing the cord with a new one, and reinstalling the cord restraint.

Sometimes electronic components fail explosively. This 8-pin IC from an infusion pump literally blew, resulting in complete failure of the pump. Though it is possible to replace surface mount components such as these, using specialized equipment, the repair can cause further damage, and also there may have been other components damaged by the failure of the IC. For these reasons and in the interest of timely repair, the whole printed circuit board was replaced.

When this feeding pump arrived in the hospital, it came to Biomed for incoming inspection. One of the first tests revealed that the pump wasn’t working at all, Further investigation showed that the screws in the AC plug that secure the conductors had not been tightened. If they had been making poor or intermittent contact, overheating and/or pump failure during use could have resulted. The factory maintenance checklist that came with the pump indicates that AC operation passed, illustrating the need for careful incoming inspections of equipment.
Rather than going through all the paperwork and delays that would have been needed to return the unit, the wires were secured in the AC plug and the unit put into service, having passed all other incoming tests.

This is the inside of the heater chamber of an infant incubator. The fan on the right blows air through the chamber. The round heater element in normally covered by a finned metal radiator assembly, and the silvery rod next to it contains a temperature sensor, used to control the heat level. Linens and other material used in caring for infants in the incubator produce lint, and normally this is cleaned out of the unit when it is cleaned after each patient. However, in this case, some of the lint was missed, which could cause both reduced heating effectiveness and also lint particles circulating within the patient compartment. The lint was removed from the chamber and a note was sent to the cleaning department to remind them to check the heating chamber.

Electrosurgery bipolar operation using forceps such as these can be used to cauterize small sections of tissue (such as blood vessels, polyps or ducts) during laparoscopic procedures. The forceps have two conductive elements that pass through a metal tube, and these elements must be insulated from one another and from the tube. If the insulation breaks down, as in the picture, a short circuit results, which can cause inadvertent heating of nearby tissue, but more importantly, reduced or eliminated cautery effectiveness. Such instruments can be repaired at the factory but not in the field, so they must be sent in to an appropriate depot.

Monopolar electrosurgery also depends on a complete circuit for operation. In this instance the conductor of the return electrode cable was severed inside the insulation, either from over-stretching or crushing, and a slight tug broke the insulation. In a pinch, the cable can be soldered together and the joint covered by heat-shrink tubing, but the cable should be replaced as soon as possible.

The note on this infusion pump says, “Leaking oil, says ‘system failure’”. A number of problems are indicated: the “system failure” message is critical, and usually means that the main CPU board of the pump has to be replaced, though it can sometimes be caused by cable connectors not making adequate contact. “Leaking oil” is a problem of a different sort. Since there is no oil in these pumps, the “leak” must have been caused by some fluid being spilled on the pump, which means that proper care is not being exercised on the nursing floor. Also, since the fluid is unknown, the pump needs to be sent for decontamination before it can be serviced.

These pictures are of two plastic shells for an infant incubator, turned upside down and showing the heating chamber. The open hole is a port that allows distilled water to enter the heating chamber via a metal nozzle, where it is vaporized and then blown throughout the interior of the incubator. The top image is of a new shell, while the bottom one has suffered extreme degradation of the plastic around the area where the nozzle was fitted. This allowed water to drip into the bottom section of the shell and eventually out onto the floor of the nursery. Biomed initially thought that a cleaning solvent may have been left in the chamber, eating away the plastic, but after communication with the incubator manufacturer, they indicated that simple moisture could cause this problem. The replacement shell was provided free of charge.

This AC power cord from a laparoscopic surgery insufflator suffered from either excessive flexing or was run over by a heavy, wheeled device like a gurney. The physical damage resulted in one strand of one of the internal conductors breaking and being thrust through both the inner and outer insulation of the cord. It presents an electrical shock hazard as well as a physical puncture hazard. The damage to the conductors could also result in failure of the device during use. The cord was replaced and staff reminded to keep cords off the floor whenever possible and away from traffic paths.

This is a fiber-optic channel used to carry laser pulses to target kidney stones, in order to break them up (lithotripsy). With normal wear and tear, or abuse, the fibers can develop internal cracks which, while not visible to the unaided eye, effectively block light transmission, making the fiber useless. Fibers must therefore be tested regularly to ensure patency. This can be done by passing an intense white light through the fiber, and looking carefully at the results. A break is indicated by light “spilling” from the break location, as in the illustration.

Immersion in liquids is generally not healthy for electronic components. This nerve stimulator was either dropped into some liquid (sometimes it’s best not to ask what), or a liquid was spilled on the unit. In any case, a residue from the liquid was left on the leads of an IC, causing failure of the device. Due to the relatively low cost of the device and the relatively high cost of replacement parts, the device was taken out of service.

Nurses love tape! This is the patient end of a nitrous oxide unit, used in this case for administration of nitrous oxide as a pain reliever for pregnant patients during labour. A rubber face mask fits over the circular opening when in use with a patient. The tape was used when the corrugated plastic tubing (which removes exhaled nitrous oxide, in order to keep it from venting into the room) broke off. Tape can be useful for emergency, short-term repairs, but it will almost certainly fail in short order, and is also a potential site for contamination as well as being aesthetically distressing.

An intake filter from an oxygen concentrator. An object of some kind was drawn into the air intake of the unit with enough velocity to perforate the filter material, resulting in potential contamination of the delivered oxygen as well as improper pressures within the device. Normally a foam filter prevents such occurrences, but in this case the primary filter was missing.

This ophthalmoscope has a rechargeable battery in the handle, and a built-in charging circuit. The head of the unit unscrews to expose prongs that plug into any AC receptacle for charging the battery. Whoever sent the unit to Biomed with this note was unaware of this, pointing to the need for ongoing staff education in relevant areas.

Fragile components, no matter how well packed, can suffer damage in transit. This replacement central station monitor display screen arrived in Biomed like this, even though the box showed little outward damage. The screen was replaced by the manufacturer who presumably took the issue up with the shipping company, armed with our photos.

Nurses and tape again. Normally power cords stay quite securely in place with friction, but this cord was loose and tape – lots of tape – provided a temporary, though unsightly, fix. Ideally, the unit would have been sent to Biomed for cord replacement as soon as the loose cord was noticed.

And still more tape! Even though these vital signs monitors have a volume control, if one doesn’t remember how to access it or is unaware of it’s existence, and the alarms and other audible signals are just too loud… tape and a few gauze pads provides a muffler. Staff education required.

When sealed lead acid batteries are overcharged and/or improperly vented, gases and heat build up inside the case and cause it to swell and warp. In extreme cases, the battery can explode from the pressure. Since they are usually inside a device, the damage caused is restricted to the inside of the device, likely causing immediate loss of function. The charging circuit of the unit may be faulty or the batteries may not be adequately designed for the environment.

When IV pump doors are left open, they are vulnerable to damage by passing carts or other large objects. The door may even be broken off it’s hinges completely, making it so that, as the note so kindly points out, the door no longer works. Replacing a door on these pumps is somewhat fiddly mechanically, but also a tedious recalibration of pressure sensors is required, so Biomeds often go around the hospital closing pump doors whenever they see them. Another case for staff education, perhaps simply in the form of an e-mail to nursing supervisors asking them to remind staff to close pump doors when not in use.

The basic troubleshooting guide that is supplied (and hopefully kept) with every vital signs monitor indicates that when the NIBP pump keeps the pressure going up and down, there is probably a leak in the system, which is almost always caused by a hole in the cuff bladder. The cuff should then be replaced. However, it’s easier just to send it to Biomed, even though the unit will then be out of service for hours if not days, as opposed to minutes if the cuff is replaced on the nursing floor.
